Jase Michael Hook 3 month old infant, passed away at Baptist Health in Madisonville, KY on Friday August 19, 2016.  He was preceded in death by his Great-great grandparents Frank and Polly Reinhard. Survivors include his mother Ashdyn Nycole Cardwell and Jason Kyle Hook of Slaughters, KY; twin sister Atlee Michelle Hook; Maternal grandparents Steffanie and David Smith; Andy Cardwell and Jaime Ashby; Paternal grandparents Dennis and Ester Hook; Maternal great grandparents Johnie Sue Reinhard, Steve and Debbie Hollowell, Donnie and Debbie Cardwell and Peggy and Alton Smith; Paternal great grandparents Dennis and Shirley Hook and Frank and Ester Springfield; Maternal great great grandparents Bobby and Grace Newton; Aunts and Uncles and cousins. Funeral services will be held at 2pm at Townsend Funeral Home in Dixon, KY on Sunday August 21, 2016. Rev. Jeff McMain will officiate. Burial will be in Wells Smith Easter Lilly Cemetery in Slaughters, KY. Visitation will be from 11am until service time at the funeral home.
